The following forms must be completed by concessionaires:

 1. Market Data forms

Pusruant to Section A28 of the Concession Agreement, the following Forms listed in Table 1 are to be completed by concessionaires and submitted to the Authority. Forms must be submitted:

• via e-mail to policy@tatt.org.tt  and
• by regular mail or by hand to the Authority's office at #5 Eighth Avenue Extension, Barataria

Market Data Forms shall be submitted no later than six weeks after the end of each calendar quarter in order to facilitate the timely publishing of the Quarterly Market Brief and the Annual Market Report. A submission schedule is included in Table 2 below.

Service Providers shall submit relevant market data forms by the following deadlines:

2. Determination 2010/01 Forms                                             
                                             
Pursuant to the Determination 2010/01, the following forms as listed in Table 3 are to be completed by concessionaires providing International Services and submitted to the Telecommunications Authority of Trinidad and Tobago:

• via email at policy@tatt.org.tt; and 
• by regular post or by hand to #5 Eight Avenue Extension, off Twelfth Street, Barataria.

 
As per section 4.1 (4) of the Determination 2010/01, forms shall be submitted no later than fourteen (14) days after the end of the period to which the information relates. A submission schedule is included in Table 4 below.
